This article is for people considering a longevity programme seriously enough to want the schedule, not the brochure. If you can commit ten days to three weeks, and you want to know what happens on each of them before you give anyone your medical history, this is the week-by-week picture.

The word longevity has been stretched to cover everything from hospital-grade medicine to scented steam rooms. The distinction that matters is simple: a physician-led programme measures you before it treats you, and it can justify every element of your protocol from your own results. Everything below follows from that principle.

Week one: diagnostics before anything else

A serious programme begins in the laboratory, not the treatment suite. The first two to three days at an accredited Bangkok clinic are given to comprehensive bloodwork, metabolic, hormonal, inflammatory and cardiovascular markers, alongside body composition analysis, cardiac assessment, and imaging where your history indicates it. Some clients arrive with a recent executive screening in Bangkok already done, in which case those results become the baseline and the diagnostic phase shortens.

Then comes the part that separates medicine from marketing: a physician sits with your results before proposing anything. The protocol you are offered on day three or four exists because of what your bloodwork showed on day one. If a clinic can hand you a treatment schedule before it has seen a single laboratory value, it is reading from a menu, and every guest is being served the same dish.

The middle of the stay: protocol, with air between appointments

The protocol phase typically occupies days four to twelve, depending on length of stay. What it contains varies with your results: infusion therapies where there is a documented reason, hormone or metabolic optimisation under specialist supervision, physiotherapy and structured training, sleep and nutrition work with measurable targets. The pacing matters as much as the content. Appointments are held to mornings where possible, afternoons are left for recovery, the river, the pool, sleep. This is not indulgence. Rest is when the physiological work consolidates, and a programme that packs four interventions into every day is optimising its invoice, not your biology.

The final days: results, plan, rest

The last two or three days close the loop. Repeat markers are taken where the protocol should already show movement. The lead physician walks you through everything in person: what was found, what was done, what the numbers say, and, crucially, what happens for the next six months. You leave with your complete file in English and a written plan covering training, nutrition, sleep and any continuing prescriptions, built to be shared with your own doctor at home.

Fourteen days, on paper

A pattern from our files, anonymised and illustrative. A client in her early fifties, a lawyer from Hamburg, booked fourteen days. Days one to three: nine diagnostic appointments, including full bloodwork, cardiac assessment and body composition. Day four: a ninety-minute protocol consultation with the lead physician. Days five to eleven: one to two appointments each morning, twelve in total, with afternoons kept free, and one full rest day mid-stay with nothing scheduled at all. Days twelve to fourteen: repeat bloodwork, a final review, and a written six-month plan with follow-up laboratory work booked near her home for month three. Twenty-three appointments across fourteen days, and not one of them after two in the afternoon.

The annual rhythm

The visit is the beginning of the programme, not the whole of it. The written plan carries you through the following months, interim bloodwork at around three and six months is reviewed remotely, and the full programme repeats annually against your baseline. 03Is a longevity programme just IV drips and supplements?
